Admirable Care is proud to be an approved provider in New York State’s Social Care Network (SCN), operating in partnership with SOMOS Community Care and Public Health Solutions. Through this initiative, we help address the Health-Related Social Needs (HRSN) of Medicaid members by providing comprehensive, community-based support that promotes housing stability, wellness, and access to essential services.

As part of our SCN service array, Admirable Care delivers Screening/Navigator, person-centered case management and a range of housing-related supports, including:

    • Initial Health-Related Social Needs (HRSN) screening and needs assessments

    • Assistance identifying eligibility for SCN housing and utility support services

    • Warm handoffs to appropriate case management or housing support providers

    • Tracking and documenting progress using SCN platforms such as FindHelp

    • Coordinated support for Medicaid members with complex social and medical needs

    • Referrals and navigation across community resources and entitlement programs

    • Follow-up and sustained engagement to ensure needs are met

    • Temporary Housing Assistance

    • Rental Subsidy Programs

    • Back Rent Assistance

    • Utility Setup – Activation Expenses

    • Utility Setup – Back Payments

    • Ongoing Utility Assistance

    • Pre-Tenancy Services: helping individuals prepare for and obtain housing

    • Community Transitional Supports (CTS)

    • CTS Broker’s Fee Support

    • Tenancy Sustaining Services: to prevent eviction and support long-term housing

    • Housing Transition and Navigation Services: tailored support during housing transitions
